During the 24/7 clinical assessment, the Immedicare clinical team will carry out where necessary, an assessment of the resident’s skin and pressure areas to support the planning and delivery of excellent care by the care home team.

Immedicare provides 24/7 clinical assessment and support for all residents who have a leg ulcer, pressure ulcer, moisture lesion or other skin damage to reduce GP and community team visits, attendance at ED and acute admission.
We offer care homes virtual clinical supervision in safe wound care management, use of wound dressings, use of pressure relieving mattresses and other equipment to reduce the risk of developing skin damage.
- Development of individual skin integrity risk assessments and prevention plans during virtual clinical assessments.
- Review of medication, nutrition and hydration status and its impact on skin damage.
- Virtual discharge pathway to support early discharge with proactive calls post hospital discharge to support staff and residents in the recovery phase.
- Training and development of care home staff in risk assessment, pressure ulcer prevention, mobility assessments and best practice on care to prevent and/or treat skin damage.
- Collaboration with local PCNs to develop effective pressure ulcer prevention pathways.
In one particular case, we had a resident with a significant wound and were concerned about the deterioration of the wound. We were unable to get Tissue Viability specialist support and so we used the Telemedicine service for advice. The nurses at the Digital Care Hub were very helpful and were able to give us the advice we needed which meant we did not need to ring for an ambulance.
