Care home residents remain one of the most vulnerable populations in our society with complex health and social care needs.

Immedicare is a digitally-enabled NHS service supporting the care home workforce 24 hours a day. We help to improve the quality of care for residents by reducing the need for referrals to other healthcare professionals, ambulance conveyance and hospital admissions.

Almost 90% of residents stay at home following a consultation with us.

Care home workers in need of additional help can call us to access an NHS professional in our Digital Care Hub through an online video link. We support homes and their residents with acute issues, falls prevention, wound care, medicines safety, nutrition and hydration and end of life support.

Many care homes struggle to attract and retain registered nurses, and our expert team of NHS nurses, pharmacy professionals, paramedics and healthcare support workers provide much needed capacity and professional support.

Immedicare - Aligning care homes to the Government’s 3 shifts

Shift 1: Moving more care from hospitals to communities

Care home staff are encouraged to call Immedicare first when they are concerned about a resident, so we can triage and assess their needs. Early identification of any issues together with a clinical response frees up health and care staff in local systems to focus on consultations that need to be face-to-face. This also helps keep residents out of hospital, which we know can be harmful for elderly people. 

Shift 2: Making better use of technology in health and care

Our service is delivered through a virtual video conferencing platform. We provide care homes with the fully supported devices and network connectivity they need to carry out high-definition video consultations. Our service can integrate with both EMIS and SystmOne (GP patient record systems), meaning the clinical team can have access to the resident’s medical history during consultations.

Shift 3: Focusing on preventing sickness, not just treating it

At Immedicare, we know prevention is key. Care homes with the Immedicare service have immediate access to clinical advice and treatment for residents, meaning we help to manage issues quickly before they worsen. We support homes to develop risk assessment and prevention plans for conditions like falls and our Pharmacy service provides medication advice to reduce future incidences. We have also provided over 3,000 care home staff with virtual training in dementia care, falls prevention, medicines administration and end of life care – empowering teams to prevent and manage residents’ needs themselves where possible.
