Our medicines optimisation service
A Consultant Pharmacist is based in the Digital Care Hub as part of our multi-disciplinary team to support medicines optimisation and offer advice and guidance in complex cases. The goal of our medicines optimisation service is to help our residents to:
- Improve their outcomes
- Take their medicines correctly
- Avoid taking unnecessary medicines
- Reduce wastage of medicines
- Improve medicines safety
Electronic prescribing is available once developments and formulary are agreed with any of our contracting organisations. This enables the Digital Care Hub non-medical prescribers to complete episodes of care which require an acute prescription. This can further support the reduction in both GP attendances and hospital admissions.

Key service features
The Consultant Pharmacist leads on prescribing, advice and guidance, mentoring and training the Non-Medical Prescribers as well as medicines safety and research.
The service offers:
- Clinical support in line with the enhanced health in care homes framework including national guidance and is responsive to local needs.
- Support for teams with patient centred approaches to reduce inappropriate polypharmacy and improve patient safety.
- Review of medications to reduce future falls risk linking with GPs and local teams.
- Support for evidence-based care in dementia including, managing behaviour that challenges, end of life symptoms and management of pain and common infections in line with antibiotic stewardship principles.
- Virtual Training for care home staff on systems and processes in line with nationally accredited courses, to reduce medication administration errors and waste.
- Virtual Clinical Case Based Discussion network which supports medication review and advanced pharmacy practice in care homes.
