Falls assessment and prevention
Falls are a leading cause of emergency hospital admissions for older people.
Falls are common in older people with up to 4,500 care home residents falling each day in England. Nearly 50% of care home residents experience at least one fall per year, and many of these incidents result in serious injury or hospital admission. Falls are the number one reason that care home residents attend Emergency Departments.
Studies have shown that 43% of falls among elderly care home residents can be prevented with the right measures, support and interventions in place.
Our service
Falls assessment
Immedicare provides care homes with 24/7 remote access to the NHS professionals in our Digital Care Hub. This means care home staff receive immediate assistance, managing falls safely when they do occur. If necessary, we provide support in the use of equipment for moving residents quickly and safely after a fall
We carry out post-fall assessments remotely, to identify any potential harm and develop an appropriate care plan. This enables local primary and community care teams to focus on those patients where face-to-face review is required.

Falls prevention
We work with care home staff to develop robust falls risk assessment and prevention plans, which considers, environmental factors, nutrition, hydration and medication reviews. When identified that medication may be contributing to falls the Pharmacy team are available to review medication and provide person centred advice to reduce risk of future falls.
