We know that many care home residents have complex medicines regimes – by the age of 80, more than a third of all people are on eight or more medicines. Additionally, studies suggest up to 17% of unplanned hospital admissions in the UK are attributable to medication-related harm and 50% of these are avoidable.

Our service

Our Immedicare pharmacy team, based in the Digital Care Hub, support residents to get the best outcome and experience from their medication. We offer advice and guidance in cases where medication needs to be reviewed and/or is identified as a potential cause of harm i.e. falls.

Improving medicines safety in care homes

We support care home residents to effectively manage their medicines. This improves outcomes for residents, and helps to reduce potential harm from medication, including falls and sedation.

Medication training for care homes

We also help care home providers to improve the quality of the service they deliver, by providing support and training for their care teams in management of medicines. Access to the Immedicare service means homes can improve their governance around medications.
