The number of people requiring end of life care in care homes has risen significantly. Providing adequate end of life or palliative care is challenging for care home staff and they can often feel unsupported to look after these residents.

Our service

The expert team of NHS clinicians in the Immedicare Digital Care Hub support care home staff to give high-quality evidence-based end of life care in the place of their choosing.

End of life care assessment

Immedicare provides 24/7 clinical assessment and support for residents approaching the end of their life, helping to reduce attendance at Emergency Departments and hospital admissions. Where needed, we also carry out reviews of the resident’s medication and/or nutrition and hydration needs, to reduce risk of harm or discomfort.

Supervised end of life care treatment

We can offer virtual clinical supervision in the safe use of medical devices, such as syringe drivers. If medicine has been reduced or deprescribed, we can monitor the resident’s response to this remotely.

Supporting discharge from hospital

Our service can act as a virtual pathway to facilitate timely discharge from hospital, where appropriate. Additionally, we will proactively contact care home staff post discharge to support staff with caring for residents approaching the end of their life.

Virtual verification of death / recognition of life extinct

Our team can provide virtual verification of expected death, which enables this difficult process to be managed in more timely way, providing reassurance to the resident’s family.
