Care Connect

Care Connect is an online portal that allows local health and care teams – GPs, pharmacies, community services and hospitals – to contact care homes directly.

Any care home with access to the Immedicare service can also use Care Connect, free of charge, via the same laptop.

This means care home residents can access local health and care services, while remaining at home. Care home staff can also use Care Connect to join multidisciplinary team meetings to discuss a resident’s care, without having to leave the care home.
