Our Immedicare service helps to alleviate pressure within Primary Care by providing timely access to an NHS clinical team, reducing GP and community referrals
How it works
The Immedicare service is available to care home staff when they need additional support for residents. We offer consultations with NHS professionals in our Digital Care Hub via video call 24 hours a day, 365 days a year.
Improving care for residents
Care home residents remain one of the most vulnerable populations in our society with complex health and social care needs.
We work as an extension of the care home team, providing a vital link between care homes and local services, ensuring the right care for residents.
Our highly specialist team provides a wide range of clinical support and is available 24/7 for any concerns. Examples, of our support include, medicines safety, falls prevention, nutrition and hydration, wound care and end of life care.
Reducing pressure on primary care and community services
Care homes call us for support when concerned about residents, following thorough clinical assessment many can be managed without the need to escalate to their GP Practice. This reduces the demand on Primary Care services.
Immedicare’s non-medical prescribing team can provide prescriptions for acute concerns following clinical assessment. Timely access to medications prevents deterioration and reduces pressure on Primary Care.
Supporting the care home workforce
Immedicare provides care homes with access 24/7 to an experienced team of NHS nurses, pharmacists, pharmacy technicians, paramedics and healthcare support workers who have extensive knowledge in urgent, emergency, community and end of life care.
Our expert team provide much needed capacity and professional support, reducing the risk of harm and giving reassurance to residents and their families.
Working in partnership with you
For over a decade we have been providing safe, high quality care to the care home sector. Working closely with our care homes and local NHS partners, we tailor our service based on local need. Our clinical consultations are supported by access to the resident’s local electronic patient record.
Following every clinical contact with Immedicare a copy of the consultation is shared electronically with the GP Practice.
Business Intelligence
Immedicare provides comprehensive reporting enabling local system oversight and monitoring of activity and impact. This provides valuable insight into local population health and care needs, supporting future service design and provision.
