Demand on health and care systems increases during the winter, due to large numbers of people suffering with the flu, Covid-19 and illnesses related to cold weather and respiratory problems. 

The prevalence of seasonal illnesses is especially common in the elderly, who in the winter months tend to suffer from a decline in health even more than the rest of the population. Whenever possible, the Immedicare service helps care home residents avoid a trip to hospital unless absolutely necessary, keeping them at home and minimising their risk of catching an illness. 

As of 16 September 2024, NHS England have written to all Integrated Care Systems about demand and capacity plans for this winter, emphasing the need to maintain and improve patient safety and experience over this period. 

Care home residents remain one of the most vulnerable populations in our society with complex health and social care needs. Our Immedicare service helps more care home residents get professional clinical care at home in their community.  

Immedicare is an essential NHS, clinical, digital health, service supporting the care home workforce 24 hours a day. We help to improve the quality of care for residents by reducing the need for unnecessary referrals to other healthcare professionals, ambulance conveyance and hospital admissions. We support with falls, medicines optimisation, post-clinical assessments, enhanced end of life care and nutrition and hydration.   

90 % of our care home residents remain at home following an Immedicare consultation.
