During the 24/7 clinical assessment, the Immedicare clinical team will carry out where necessary, an assessment of the resident’s nutrition and hydration needs to support the delivery of excellent care by the care home team.

Immedicare provides care homes with 24/7 clinical assessment and support for all residents who are dehydrated or malnourished to improve outcomes and reduce attendances at ED and acute admissions.
We offer virtual clinical supervision in the safe use of oral nutritional supplements (ONS) and medical devices used for enteral feeding, to both reduce the risk of malnutrition and dehydration or following its development.
- Virtual discharge pathway to support early discharge with proactive calls post hospital discharge to support staff and residents in the recovery phase.
- Training and development of care home staff in risk assessment, nutrition, hydration and best practice to identify, prevent and/or treat malnutrition and dehydration.
- Follow up calls to ensure intended outcomes of prescribed supplements are being achieved.
- Collaboration with local PCNs to develop effective malnutrition and dehydration prevention pathways.
