Care home residents often have complex medication regimes – by the age of 80, more than a third of all people are on eight or more medicines. Additionally, 20% of hospital admissions in the over 65 age group are caused by adverse effects of medicines, which two thirds of are considered preventable.1
Our service
The Immedicare Pharmacy team support safe management of medicines, undertaking reviews and offering advice and guidance to care homes, focusing on reducing the risk of error and potential harm from medication regimes which may lead to falls or other adverse effects. This service helps residents living in care homes to get the best outcome and experience from their medication.
Non-medical prescribers in the hub are also supported by the pharmacy team to work within a robust evidence-based framework.
Medication training for care homes
We also help care home providers to improve the quality of the service they deliver, by providing support and training for their care teams in management of medicines. Access to the Immedicare service means homes can improve their governance around medications.
