I’m Dawn, the Lead Pharmacist in Immedicare’s Digital Care Hub. I support care home residents to get the best experience and outcomes from their medication, including trying to reduce the risk of potential medication-related harm. I joined Immedicare in April 2024 – I was previously working as Deputy Chief Pharmacist and Medicines Safety Officer at a mental health trust. Throughout my career, I’ve always been particularly interested in frailty and mental health. 

Our Immedicare team is available 24/7 to provide advice to care homes and their residents via video call. We work closely with care home teams and clinicians in their local area, including GPs, who we try to build strong relationships with. 

I work with Gemma, our Lead Pharmacy Technician, to improve medicines safety in the care homes we work with. Currently, we are focusing our efforts in four main areas: 

  • Reviewing resident medication after a fall 
  • Supporting residents following a medication incident 
  • Supporting non-medical prescribers to enable timely access to medications for residents 
  • Providing medication training for carers and clinicians 

My role includes directly carrying out reviews with residents as well as making decisions about how we want the medicines aspects of our service to work moving forward. 

I really enjoy working as part of the Immedicare team, as we use our full range of expertise across the Digital Care Hub to deliver a service that makes a real difference to care homes and their residents. It feels genuinely meaningful to advocate on behalf of residents and help with reducing the health inequalities they experience.
