The Conifers Rest Home Case Study

The Conifers Rest Home is a small, homely residential home which specialises in dementia care. The home has been utilising Immedicare’s telemedicine service since August 2017, to provide up to 18 residents with 24/7 clinical support.

Debra Buxton, Owner of The Conifers, describes the issues the home regularly faced: “When a resident was unwell or had a fall, we felt it was a long process for them to be assessed. We would either have to call a GP and wait for them to arrive or call the out of hours number. They would often have to go to hospital and wait in A&E when this wasn’t necessary, as we had no alternative.”

The home became part of a pilot scheme after being offered the telemedicine service by their local CCG. “We loved the idea of being able to be have a clinical team assess our residents face-to-face so quickly and easily” says Debra.

Immedicare provides an easy to use telemedicine service into care and residential homes, giving staff and residents 24/7 365 access to a highly skilled multidisciplinary clinical team from a range of specialist fields including; urgent, emergency, community and end of life care. The team at the Digital Care Hub have full access to individuals’ electronic care records, allowing them to provide comprehensive clinical assessments, guidance on condition management, and ongoing monitoring.

“Initially some of our staff were wary of using the technology, but they picked it up easily as it’s so simple to use. We all have great confidence in the telemedicine” says Debra. “Immedicare has reduced the number of hospital admissions in our home and has given us the ability to be very responsive to our resident’s needs, as the telemedicine speeds up the process of getting a clinical assessment. Now we can call the Digital Care Hub, who can assess the resident, organise a prescription for us and even get an ambulance to the home if necessary.”

Our night staff particularly benefit from the service, as they can’t ring a GP during the night, so the only option is to ring out of hours. The telemedicine service has been a lifeline for the night staff.”

“We have a resident who has advanced stage dementia. She fell and bumped her head, and usually would have had to go to hospital. However, called the Digital Care Hub, who arranged for a paramedic to visit us and glue her head, so she didn’t have to go to hospital. Another one of our resident’s turned yellow, so we called the Digital Care Hub and because they could see the resident, they called an ambulance straight away. This saved time, as we would have had to wait for a GP before Immedicare.”
