Immedicare, a West Yorkshire based Telemedicine service provider has been commissioned by Liverpool Clinical Commissioning Group (LCCG) since March 2017, providing a 24/7 video based clinical response service into nursing and residential care homes across Liverpool. The majority of Liverpool’s 56 Care Homes have now adopted Telemedicine and three purpose built Intermediate Care Units also use Telemedicine, with a further two newly built hubs coming on-board in Winter 2019.
Immedicare is a leading provider of video-enabled clinical healthcare services, delivered into care homes across the UK. Using aa proven and proactive engagement approach, coupled with close integration with local service providers, Immedicare has been shown to reduce ambulance conveyances and A&E attendance rates, acute hospital admissions as well as both in and out of hours GP visits.
The service has been deployed as part of the wider Liverpool Care Home Improvement Strategy, which is a joint strategy between LCCG, Liverpool City Council and the care home community, and is dedicated to improving care in care homes, through innovation, across Merseyside. This will be enhanced by the implementation of the Enhanced Health in Care Home Framework implementation across the City in a phased approach over the next year.
The team at Immedicare have worked closely with LCCG and the Care Homes to monitor usage and provide monthly dashboards on use, obtain regular feedback from users, drive system improvement and evaluate the impact on Homes who have truly adopted the Telemedicine.

The Telemedicine service has helped avoid unnecessary unplanned hospital visits and makes best use of GP capacity as the Immedicare nurses can request prescriptions from the residents GP, if medication is required. Telemedicine reassures the homes, particularly out of hours, about the resident’s wellbeing and treatment options.
